Wendy has recommended some on the other thread in the confidence section the 'Feel the Fear' book - I've ordered a second hand copy off Amazon (cheap skate me lol!)
Another excellent book I used when dealing some personal issues was 'Mind over Mood' it gives you kind of worksheets to deal with and breaks down your issues (mine were like panic attacks) into 'hot thoughts' and then addresses how to manage them. - It;s probably geared more towards dealing with depression but some of the exercises I still use now when I'm feeling anxious and it really helps.

I know I keep on mentioning it but I got a lot from 'Ride with confidence'. It has five chapters each by a different author. One of the chapters is on NLP, one on hypnotherapy, and so on. It's done a lot to help me and some friends who were also suffering. I would recommend it highly.
